After 12 months of your rhinoplasty operation, almost all of the swelling in your nose will disappear. At this stage, you should be examined by your surgeon to evaluate your surgical result and to check the scar tissue.

What are the risks involved with face and Rhinoplasty?

The cartilages in the structure of the nose undergo a change with aging. Therefore, just aging causes our nose to sag a little. Trauma of these cartilages due to an accident or surgery, accelerates the changes that come with aging (apoptosis). Vision Correction with Intraocular Lenses

Intraocular lenses (IOL) are artificial lenses that are placed in the place of a clouded one during cataract removal.

Cataracts are one of the main causes of vision impairment and blindness. In different countries, this diagnosis occurs with a frequency of up to 30%, mostly elderly people who are ill. To date, there is only one effective way to restore vision in cataracts — surgical. The clouded lens is removed, replaced with an artificial one – an intraocular lens (IOL). And, although the method of cataract treatment is the same in all cases, the patient has the right to choose the type of IOL after consulting with a doctor beforehand.

What are intraocular lenses and why are they needed

The human eye is a perfect optical system, far superior in capabilities to the most sensitive modern cameras. Each element in it is in its place and performs a special function. One of the most important details of this complex system is the lens. Externally, it is a biconvex lens, the function of which is to refract the light beam coming through the pupil and focus it on the retina.

With a disease such as a cataract, the lens becomes cloudy and eventually loses its function. As a result, visual acuity decreases, a person ceases to see objects clearly, color perception worsens. In the later stages of the disease, complete blindness sometimes develops.

Unfortunately, it is impossible to restore the affected lens. Drops and other conservative remedies only slightly slow down the progression of cataracts in the initial stages. Medicine knows only one radical method of treatment that helps to restore normal vision to the patient — a surgical operation to install an artificial lens of the eye.

Another name for such products is intraocular lenses. They are made of a material that is biologically compatible with the tissues of the eye (acrylic, silicone), and therefore the risk of rejection, allergic and inflammatory reactions is minimized.

Structurally, the IOL is a lens with a diameter of about 5 mm with shackles for fixing. If all these parts are made of the same material and are made as a whole, such a product is called a monoblock. Modern IOLs are soft, which allows minimizing the degree of tissue injury during surgery. The twisted lens with a special tool – an injector – is implanted through a micro-incision, after which it straightens itself, taking the desired position. Models of previous generations were made of rigid materials. To install them, a large incision was required, after such an operation, stitches had to be applied. Now such problems are a thing of the past.

Different types of IOLs are used in ophthalmology. All these products can be divided into two large groups — phakic and aphakic. The former serves to correct myopia, hyperopia, and astigmatism and, thus, are used as an alternative to glasses and contact lenses. When they are installed, the lens is not removed. In the surgical treatment of cataracts, only aphakic IOLs are used. So we will talk exclusively about this type of intraocular lenses. But even within this group, there is its classification.

Types of IOLs, their differences, and advantages

The first IOL implantation operation was performed in the middle of the XX century. This was a real breakthrough in ophthalmic surgery, however, the material of the artificial lens itself, the design of the product, and the technique of surgical intervention were far from perfect. The products took root well, but patients often had inflammatory and other complications, and the fixation of the implant was not reliable enough. In addition, the first IOLs could not correct refractive errors (nearsightedness, farsightedness, and age-related presbyopia, astigmatism).

The most common classification of intraocular lenses is based on the sign of focality. To understand what this means, you need to remember the properties of the lens. This natural “lens” has elasticity and the ability to change shape according to the focal length. It becomes more or less convex depending on whether we are looking into the distance or observing a nearby object. In other words, the lens can adjust to the desired distance, so a healthy eye sees equally well both near and far (this ability is called accommodation). An artificial intraocular lens made of synthetic material, of course, does not have such properties. Its ability to focus at different distances (pseudo-accommodation) depends on the number of optical zones. From this point of view, monofocal and polyphocal IOLs are distinguished, as well as modern EDOF lenses, which in this respect are practically not inferior to a real lens.

Monofocal IOLs

A monofocal, or single-focus, the lens has only one focus zone, most often the far one. With its help, a person can clearly distinguish objects in the distance, but cannot see up close. To read, use a smartphone, sew or knit, you have to use “positive” glasses (which are worn with farsightedness).

A less common option is lenses with a near focus. They are suitable for activities that require clear vision at a short distance but do not work well at long distances. After installing such lenses, a person can easily type SMS or read the fine print on the product packaging, but for walking or driving a car, “minus” glasses will be required.

This limited functionality is the main drawback of mono-focal lenses. As for the advantages, it can be noted, perhaps, only a lower cost compared to more advanced models.

Polyphocal IOLs

This is the next generation of intraocular lenses. They have not one, but several optical zones, each of which is responsible for focusing at a certain distance. Such lenses allow you to see both near and far, and at medium distances. The more focus zones (and there can be two, three, or more, respectively, IOLs are called bifocal, trifocal, multifocal), the wider the range of visual perception. At the same time, you do not need to additionally use glasses.

To correct the shortcomings of previous IOL models and create an artificial lens that is as close as possible to natural properties, the developers, after years of searching, came to the EDOF technology. To date, this is the newest trend in ophthalmology, actively gaining strength.

EDOF lenses

The abbreviation EDOF stands for and translates as “extended depth of focus”. The essence of the technology is that a wide range of the surveyed space is projected into a single focus area (as opposed to several separate zones in polyphocal IOLs). Dietary Guidelines For Post Bariatric Surgery

The success of your gastric sleeve surgery will depend on your ability to stick to a healthy diet and lifestyle.

Week 1 Post Operation

Duration: This stage will start when you are in the hospital and last 5 – 7 days (up to 14 days)

Recommended Intake: Clear liquid intake goal is 64 ounces (2 Liters). Minimum of 48 fluid ounces (1.5 Liters) per day. (Clear broths/bouillon/chicken/beef/vegetable).

Clear liquids can include:

Water;
Decaffeinated coffee or tea including herbal, fruit, and green;
Clear broth such as chicken, beef or vegetable;
Diluted fruit juice such as apple, grape or cranberry;
Diluted sugar-free squash;
Sugar free drinks, flavoured water.
You should avoid:
Caffeine – may contribute to acid reflux and dehydration;
Sugary drinks – may result in nausea, fatigue, diarrhoea, and even vomiting;
Sugar is also full of empty calories;
Carbonated drinks – may contribute to bloating;
Milk or dairy products.

Week 2-3 Post Operation

During the second week after your sleeve gastrectomy, you will continue to drink plenty of clear fluids whilst also incorporating into your diet fuller liquids that are rich in protein.

Your appetite may increase but your digestive system is not yet ready to handle solid food. You should fill up on a variety of nutritional liquids.

Full liquid choices in addition to your clear liquids include:

Low fat, lactose free milk. Milk can be flavoured with a low-calorie hot chocolate;
Homemade smoothies.( up to 2 cups);
Slimming drinks and shakes;
Yogurt drinks;
Sugar-free, non-fat plain Greek yogurt, frozen yogurt;
Sorbet;
Plant Based Milk ( Soy or Almond or coconut);
Protein Powders;
Natural Applesauce;
Vegetable juice;
Hot cereals like oatmeal/ cream of wheat;
Sugar-Free Pudding;
Soups (choose higher protein soups like bean soup,chunky soups that need to be blended before eating).

Additional foods to avoid include:

Sugary foods;
High-fat foods such as whole-milk yogurt;
Food that has any lumps;
Avoid all carbonated beverages, sugar and coffee after surgery.

BREAKFAST

1 oz. packet high protein oatmeal made with almond milk

One scoop unflavored protein isolate added in

MID-MORNING

Protein shake (minimum of 20 grams of protein) – clear or thick shake

LUNCH

4 oz. Greek-style yogurt blended with almond milk (chose a yogurt that has more protein than sugar)

Week 4-5 Post Operation

It is important to only eat until you feel satisfied and not more than that;
to eat a high protein, low sugar, diet;
Never eat and drink at the same time;
Never eat more than 4 ounces of food at once;
Chew your food thoroughly;
Aim for 800 calories daily;
Never eat before bed;
You need to ensure you eat 60 grams of protein each day so include protein in each meal. You can try adding milk when you blend your food, ensuring that it is free of lumps and a yoghurt like texture;
Try to eat small portions slowly. Re-introduce new foods slowly, giving your body time to react to each new food so that you can identify any foods that are disagreeable and cause gas, stomach upset, or diarrhoea.

Pureed foods can include:

Mashed fruit – bananas, avocados, and canned fruit. Be careful of their sugar content;
Blended steamed fish in a low-fat sauce;
Canned tuna and salmon;
Mashed sweet potato;
Hummus;
Low-fat cottage cheese and soft cheeses;
Liquidised meat or fish stew;
Liquidised steamed or boiled vegetables;
Thick blended soups;
Scrambled eggs;
Ground chicken or beef;
Cottage Cheese;
Tofu;
Veggies( Mashed, canned, and slow cooked to tenderize);
Greek Yoghurt.

Additional foods to avoid include:

Food high in starch such as pasta, rice and bread;
Tough, raw vegetables such as broccoli, asparagus, celery, and leafy greens;
Skin and seeds from vegetables and fruits;
Fatty foods including oils and butter;
Spicy food.

BREAKFAST

2 scrambled eggs & 1 oz. string cheese (3 oz.)

Week 6-7 Post Operation

By week six, you can eat solid food safely. You can use your new eating plan as a template of healthy eating for life. Your new diet needs to be low in fat, low calorie, and portion controlled. Keep introducing new foods and monitoring any reaction to them.

Avoid empty calorie foods that do not have any nutritional value. Try to keep to three small meals a day and minimal snacks. Don’t forget to keep yourself hydrated.

Healthy diet foods include:

Lean meat, poultry, and fish;
Low-fat dairy products;
Steamed or raw vegetables and fruits;
Good starches and whole grains;
Fats in small quantities.

Additional foods to avoid include:

High calorie foods and drinks;
Sugary sweets and soda;
Long term diet tips following gastric sleeve surgery;
You will need to continue eating a healthy, high protein, low fat, calorie-controlled diet for the rest of your life to achieve and maintain your weight goals.

Some top tips include:

Keep to three small meals a day;
Have protein in each meal and eat it first;
Introduce new foods one by one;
Take a tablet form daily vitamin and mineral supplement as directed by your bariatric team. You will also require vitamin B12 injections (1mg) once every three months;
Eat slowly and chew your food thoroughly. The more you chew, the easier your food is to swallow and digest;
Plan your meals so you are not tempted by off-diet foods;
Drink plenty of water and always have some water to hand;
If you feel hungry between meals try taking a drink first as your body may be mistaking hunger with thirst. If you are still hungry then have a small snack such as a piece of fruit or a low-fat yoghurt;
Ask for half portions at restaurants – it may be discounted and it will prevent you from being tempted to overeat.

Foods to avoid:

Food with empty calories – are harmful. Avoid them because they have little or no nutritional value.
Alcohol – is high in calories and your alcohol absorption will dramatically increase after surgery.
Fizzy drinks – as they cause bloating and can increase your small stomach size.
High fat foods – they may make you feel nauseous and will not help you to lose weight.
Tough meats – are hard to chew and hard to digest.

Things to avoid:

Avoid eating and drinking together – wait at least thirty minutes between eating and drinking. Drinking fluids with meals may lead to an overfull stomach and vomiting. It can also stretch your stomach and “wash” your food through too quickly so that you don’t sense the early signs of fullness and may lead to over eating.
Avoid overeating – stop eating when you feel satisfied. Overeating will stretch your stomach pouch and may make you vomit.

Nutrition Intake After Bariatric Surgery

The success of bariatric surgery depends on your post-development diet plan. Post-surgical nutrition has two goals that are listed below,

The nutrition program after bariatric surgery consists of 3 stages;

1. Clear liquid period (2 weeks)

2. Puree period (2 weeks)

3. Solid Diet

In order to protect the patient after being discharged from the hospital, the person starts to their food consumption with liquid diet after the last leakage test. The liquid diet applied during the first 14 days post-operation, all the patient drinks should be in liquid form, dairy products and carbonated drinks should not be consumed. Carbonated beverage is prohibited for life in patients who have undergone sleeve gastrectomy.

The reason for this is that carbonated drinks dilate the stomach pouch and incurable halitosis (bad breath) . The free consumption within the clear-liquid diet can be shown as water, herbal tea, beef broth, chicken stock, vegetable juice, 100% organic fruit juices, wishy-wash black tea and homemade sugar-free compote.

The puree period can start in a controlled manner after the completion of the 2 weeks liquid diet period.The determinant here is the weight and the diet of the patient, before the operation. Since the feeling of hunger is prevented, the patient receives the calories his/her body needs with a small portion of food and does not feel hungry.

During the puree period, a program that can be implemented life-long is provided. İnclusive of 5 meals as 3 main meals and 2 snacks is given. Patients start a puree diet planned by a dietician starting the second week post- operation till the end of the fist month. This period is the sensitive in terms of which the food consumed must pass through the blender, milk and dairy products can be consumed without lactose.

Protein intake is a significant factor after sleeve gastrectomy operation. Adequate vitamin, mineral and protein intake plays an important role in this process and its continuation. Individuals that have undergone the operation, when the food consumption is not carefully planned and nutrition is not regulated as it should, hair loss, nail breakage and non-growth problems and dryness of the skin are seen. For the reasons listed, eggs, meals containing minced meat and protein powder are used in the pure phase. As of the 10th day, the protein powder that will be used until the 3rd month is also crucial for the benefit of the patient.

Another important element planned in the nutrition of the patient is the solid-liquid separation aimed to be made for life. There are 3 important reasons for this practice, which aims to put half an hour between each liquid and solid food;

Protein deficiency,
Enlargement of the stomach pouch
Dumping syndrome.

In the patient’s already restricted food intake, when both liquid and solid are consumed together, enough protein intake cannot be provided, solid foods swell together with liquids would enlarge the stomach by taking up more space in the stomach pouch, and solid and liquid foods combined will come to the intestine faster and sugar is absorbed faster than normal.

Patients who have completed their puree period without any problems and that are entering the 1st month of solid period post-operation. For the duration of the solid period, food consumption must be planned to prevent blosiveness and within the high protein content and rich in nutrients as well. The foods prohibited for this period can be simplified as all meat and chicken, bread and bread products, flour products, and types of rice. Prohibition of these nutrients is primarily to protect patients, rather than the calorie intake. Since all meat and chicken are very rich in fiber, they strain the stomach pouch. The rest of the prohibited foods such as flour products and rice, on the other hand, swell in the stomach and cause discomfort in the stomach pouch of the patient who is already full within small portions. In between the 3rd and 12th month post operation within blood tests, vitamins and minerals are checked, and nutritional and supplementary vitamins are given in case of any deficiency.

Nutrition Recommendations After Surgery

Do not consume solid and liquid foods and beverages together at meals.
Fluids should be taken 30 minutes before or after meals. Liquid consumption during meals fills the small stomach pouch and can cause vomiting.
Although your stomach is reduced in volume, frequent consumption of high-calorie foods can cause weight gain rather than weight loss.
Alcohol contains high calories and encourages your appetite, so it should be avoided.
Foods should be consumed slowly, with small bites and chewing well. A meal should last 30-45 minutes on average.
If the food is not chewed enough, it may cause pain, discomfort and vomiting by obstructing the stomach outlet.
Pay attention to the feeling of your body!
Stop eating and drinking when you feel satiety and pressure in the middle of your stomach.

Exercise must be carried out! Starting from the first month, with a light-medium tempo, a walk should be done during the period when the patient feels well.

Nutrients That Should not be Consumed Post Operation

Legumes (dried beans, chickpeas, lentils, etc.) should not be consumed for an average of 6 months to prevent gas.
You should not consume cauliflower, broccoli, cabbage or vegetables for a month. Consumption be started in a controlled manner after the first month.

What is root canal treatment?

The part we call pulp is the space in the middle of the tooth. Which is located under the enamel and dentin layer. The portion of this space at the crown of the tooth called the pulp chamber. This gap, which continues along the root of the tooth, becomes a small channel by narrowing.

Molars are located in the posterior part of the mouth and usually have two or three roots. Other teeth have a root too. Where vessels and nerves are extending into the jawbone that divides into branches. These veins and nerves nourish the pulp through the bloodstream and protect it against infection.

Nerves are just a part of the living tissues that make up the pulp. Bacteria are the cause of the infection in the pulp. The bacteria move towards the pulp through tooth decay or fracture, the bacterial invasion first begins in the pulp chamber and washes the defenses. It then proceeds along the root canal and completely damages the pulp. Root canal treatment exists to save the decayed or infected tooth.
